UPMC Community Health team is Building a Database

Dear PASNAP members,

 

UPMC Children’s Hospital of Pittsburgh Community Health works together with hospital, other healthcare, and community partners to improve health outcomes, reduce health disparities, and elevate quality of life for children and families in the communities where they live, learn, work and play.

 

One area of focus for the Community Health team is our Healthy Schools Programs, which works with school staff and administrators to create a healthy environment for students to thrive. In an effort to help improve communication and efficiencies among school nurses and primary care physicians, the Community Health team is creating two databases with contact information for school nurses and primary care physicians. The goal is to provide a database to each group to facilitate the communication process around patient care. We are asking Allegheny County school nurses to complete a form below to help build our database.
