Allegheny County School Nurse Resources from UPMC Children's Hospital of Pittsburgh's Healthy Schools Team

Dear Allegheny County School Nurses,

 UPMC Children’s Hospital of Pittsburgh Division of Community Health aims to improve child and family health through community and hospital based initiatives. Through our Healthy Schools Program, we have been working with school nurses in several Allegheny County School Districts to share resources and tools to create healthier school environments.

With input from school nurses, we have put together a few tools to help improve collaboration and coordination between school nurses and clinical providers. We are disseminating these tools through a few different channels, so apologies if you receive this information more than once. 

 Clinical Provider Database--We have developed a database of local primary care providers, including both UPMC and non-UPMC affiliates. All of these providers participate in the PA Pediatric Health Network (PPHN). The attached spreadsheet lists practice names and locations, care coordinators' contact, phone #, and fax line. The information will be updated annually. PPHN providers are receiving a similar list with school nurse information.

 Communication Guidelines --The attached guidelines will aid in gaining access to immunization information and medication/treatment plans from UPMC providers.

 Authorization for Release of Protected Health Information--The attached PDF can be shared with student’s parents/guardian when requesting health information from UPMC.

 Children’s Hospital of Pittsburgh Subspecialty Guide--The attached subspecialty guide outlines some of the most common services offered at Children’s that you may encounter with your students, along with the physician office/clinic contact information and resources. You may find some of the resources included useful for your practice as a school nurse, and even for student education.

 The Children’s Hospital Community Health team hopes that the attached materials assist in caring for your students.
