Your Feedback is Needed on the ADA’s Diabetes Medical Management Plan

The American Diabetes Association®’s (ADA)Safe at School® Working Group requests your feedback and completion of survey below asking about your experience using the ADA’s Diabetes Medical Management Plan (DMMP), updated in 2022. The DMMP provides detailed guidance and medical orders for diabetes management at school. Survey feedback and results will be used to revise and update the DMMP.


Who can take this survey? 
- Any diabetes provider (physician, including trainees, nurse practitioners, physician
assistants, diabetes care and education specialists, dieticians, social workers, nurses,
and other patient-facing health care workers) who have personal or center experience
with the DMMP.
 - Parents or legal guardians of children with diabetes who use a DMMP.
 - School nurses who assist children with diabetes who have a DMMP.


A few things to know:  
 This voluntary and confidential survey will take about 15 minutes to complete. 
 All responses are anonymous, meaning they cannot be linked back to you.
 There are no anticipated risks to completing this survey.
