Healthcare providers, schools, local health departments who are assisting displaced families from Puerto Rico in obtaining immunization records may be able to do so by accessing the Puerto Rico Immunization Registry's (PRIR) public portal which is accessible HERE. Access to the public portal requires a high level of cooperation from the parent or guardian since the registry user must provide the child's social security number or Medicaid Id. If you are unsuccessful in obtaining an immunization record through the public portal please contact the VPDP (Vaccine Preventable Disease Program/Dept of Health) office in your state.
